Take these steps for your children:

  1. Speak with someone you trust about caring for your children if you cannot.
  2. Explain the plan to your children.
  3. Write down the information your standby guardian will need:
    • Children's school
    • Children's doctor
    • Health insurance
    • Parent A-Number
  4. Complete the legal form.
  5. Witness, sign, and notarize the form.
